Issuing of Certificates

 

【Notice】About Certificate Issuance

 Since there are a great number of applications we receive March every year, please give yourself an extra time than usual to request your certificate.

 

In principle, certificates will be issued as follows. Certificates cannot be issued on the day of request.Please make a request with time to spare, as it takes time to issue a certificate.

If a certificate other than those listed below is needed, please visit the Graduate School of Engineering Student Affairs Division.

The service of issuing certifications is stopped during weekend, public holiday, the year-end and New Year holidays, and Summer holiday (about in the middle of August).

Requesting Certificates

Request by post or at the office.
* Requests cannot be made by telephone, fax, or e-mail.

Complete the “Request for Certification ” (Page 1: Request form; Page 2: Example of completed form), and enclose a self-addressed and stamped return envelope. Also include a photocopy of your personal ID (such as a driver’s license) so that your identity can be verified.

  • * If you do not know your student number, you may leave that space blank.
  • * If you are requesting from overseas, include an international reply coupon. (There is no charge for issuing the certificates.)

Obtain a “Request for Certification” at the office, and fill in the necessary information. Bring with you a photocopy of your personal ID (such as a driver’s license) so that your identity can be verified. If you want your certificate delivered by post, submit a return envelope (with your address written and stamp affixed) along with your request form, to the office window.

  • ●Location: Graduate School of Engineering Administration Building (U1M) 1F, Student Affairs Office

[Important information concerning request by proxy (at office or by post)]
If you are a graduate of the School/Graduate School of Engineering and you are unable to apply in person, you can ask a proxy to apply for you instead. In such a case, please note that:

  • (1)It needs to submit an original paper of “letter of proxy ” written signature of the graduate and put personal seal of the graduate on to the registrar. Bring it to the office, or if you are sending it by post, enclose it along with the application.
  • (2)The proxy should bring a photocopy of his/her personal ID (such as a driver’s license) to the office, or if you are sending it by post, enclose it along with the application.

[Note for those whose name has changed after graduation]
If your name has changed after graduation due to e.g. marriage, you must submit a separate certificate describing the change (an abstract or copy of your family register, showing both your previous and current name) for identification, as the name on your current ID will not match that on our records.
